RISE is available to Vaya Health recipients in Buncombe County.

What is RISE?

RISE is a mobile, multidisciplinary outreach service that supports people living with both substance use and mental health challenges.

Our RISE Team meets clients where they are – literally delivering care in homes, parks, shelters or on the street.

Who We Help

We support adults (18+) in Buncombe County who:

  • are struggling with opioid or other substance use
  • have co-occurring mental health needs
  • may be unhoused, recently discharged from jail or the hospital or at risk of crisis

  • benefit from coordinated, team-based care in the community
  • are ready – or not yet ready – to start their recovery journey

  • Navigating courts, probation or reentry after incarceration

Our Approach: Community, Compassion, Connection

  • Services where you are — We provide care in homes, parks, shelters and streets.

  • No or low barriers to access — You don’t need to be “ready” for treatment. Our team uses a harm reduction model that honors your goals and autonomy. We focus on safety, trust and progress – not perfection.

  • Whole-person approach — Support includes medical care, mental health counseling, peer support, case management, housing help and more.

  • Licensed substance use and mental health clinicians — Provide integrated therapy and recovery planning

  • Psychiatric providers and nurses — Support medication needs, physical health and psychiatric care

  • Peer support specialists — Individuals with lived experience who walk alongside you with empathy and hope

  • Housing and vocational specialists — Help secure housing, employment and income supports

  • Case managers — Coordinate care and advocate for your needs across systems

  • Criminal justice liaisons and recreational therapists — Help with reentry, structure and building a meaningful daily life
